Participant Agreement and Release of Liability

1The parties and the program

This agreement is between me, the participant, and Truth as Leadership Coaching LLC of Longmont, Colorado (the “Company”). It covers every workshop, class, coaching session, or other event that I register for with the Company, in person or online, together with any related follow-up sessions (together, the “Program”).

Programs are facilitated by Christoph Roman Streubert, a HeartMath® Certified Trainer, or by a facilitator the Company names.

2An educational program

The Program is educational. It teaches self-regulation, resilience, and personal development techniques, including techniques developed by HeartMath. It is not medical care, psychotherapy, counseling, or any other form of diagnosis or treatment, and it does not replace care from a licensed medical or mental health professional.

The facilitator acts as a trainer and coach. He does not act as a physician or therapist, and the Program creates no doctor-patient or therapist-client relationship.

3My health and medical care

I will continue to rely on my own licensed providers for medical and mental health care. I will consult them before I change or stop any treatment or medication.

If I have a medical or mental health condition that could affect my participation, I will speak with my provider before the Program. In an emergency I will call 911 or my doctor.

4HeartMath technology

The Program may include demonstrations of emWave® or Inner Balance™ heart rhythm feedback technology. These are educational tools. They are not medical devices, and their readings are not to be used for medical diagnosis.

5Voluntary participation

My participation is voluntary. I may decline or pause any exercise at any time. I am responsible for my own physical and emotional wellbeing, and for my decisions, actions, and results during and after the Program.

6Assumption of risk

The Program may include breathing techniques, guided attention exercises, personal reflection, and group discussion. I understand that these activities may bring up physical sensations or emotional discomfort, such as lightheadedness, strong emotions, or stress.

When a Program takes place in person, I also accept the ordinary risks of attending, including travel to and from the venue and use of the venue. I voluntarily assume these risks.

7No guarantee of results

Results differ from person to person and depend on my own practice. The Company makes no promise of any specific outcome.

8Release of liability

To the fullest extent permitted by law, I release and hold harmless Truth as Leadership Coaching LLC and its owner, facilitators, contractors, and agents from all claims, demands, losses, and damages that arise from my participation in the Program, including claims based on ordinary negligence.

This release does not apply to gross negligence, reckless conduct, or intentional misconduct, or to any claim that the law does not allow me to release. It also binds my heirs and personal representatives.

9Confidentiality and respect

I will keep personal information shared by other participants confidential, and I will treat everyone in the Program with respect. I understand that the Company cannot guarantee that every participant will do the same.

The Company will treat my personal information as confidential. It will use it to run the Program and to stay in touch with me, and it will disclose it only where the law requires.

10Photos and recordings

I will not record any part of the Program. The Company may take photographs during in-person sessions and may record online sessions. I can ask to be left out of any photograph or recording, and the Company will honor that request.

The Company will ask for my separate permission before it uses my image, name, or words in its marketing.

11Program materials

The Program materials, including HeartMath techniques and content, are protected by copyright and trademark. They are provided for my personal use. I will not copy, distribute, or teach them commercially.

12Conduct

The Company may ask a participant to leave if that participant's behavior disrupts the group or puts others at risk.

13Governing law

The laws of the State of Colorado govern this agreement. If a court finds any part of it unenforceable, the remaining parts stay in effect.

14Acknowledgment

I am at least 18 years old. I have read this agreement, I understand it, and I agree to it voluntarily.

This agreement applies to each Program I register for. Checking the acknowledgment box and signing in a registration form has the same effect as signing this document by hand.
